Care manager - Adults - Bromley

Our client is a local authority looking for a Qualified Social Worker to work in Care Management team.

To be considered for this role you must have: -

*A degree in Social Work
*Hold a full HCPC registration
*Two years post graduate experience in a relevant team
*Experience of working in delivering client care, within either a care home, hospital or care home environment.
*Be eligible to work in the UK
*Driving licence and business insurance would be beneficial
*DBS update is preferred by client

The role will involve: -
*Undertaking client assessments, in accordance with legislation and company values the client care pathway ensuring all focus on placements/ clients/ patients, whether home based or in care home, hospital is delivered within agreed time lines
*Accountable for client/ patient set up from point of transaction/ sale/ setting them up on your system, through to the client start date, including paperwork and systems data
*Maintaining care-related documents, ensuring all processes completed
*Planning the case load for care related activity from point of assessment through to service end)
